Aviation Activity Acknowledgment
Helicopter flight is an inherently complex activity. While modern aircraft are reliable and our crews are highly trained, mechanical failures, weather changes, bird strikes, and other events can occur. You acknowledge that:
You are voluntarily participating in a helicopter flight as a passenger.
You have considered the risks and chosen to fly with full knowledge.
You have disclosed any medical conditions that may affect your ability to fly.
You will follow all safety briefings, crew instructions, and aircraft procedures.
